This is an exciting opportunity to support young people as they transition to independence, ensuring they are safeguarded, supported and empowered to achieve their potential. As an experienced social worker in the 16+ Team you will: Work with children and young people (aged 16+) to assess needs, provide support and develop care and pathway plans.
