Your new company Sandwell Council are looking for an experienced Mental Health Social Worker to join our busy and thriving Hospital team. The team is led by a Team Manager with a wealth of experience who believes in support and development and healthy work / life balance. Your new role You will assess the needs of people with acute, complex or enduring mental health issues and the needs of carers. You will monitor, review and evaluate services in order to ensure that they are provided effectively in accordance with need, together with the service users and carers. Formulate and implement care plans appropriate to their needs. You will have a good working knowledge of the Care Act 2014, Mental Capacity Act 2005 and up-to-date safeguarding.
