Job description: We are looking for a skilled and motivated Social Worker to join our Hospital Discharge and Discharge to Assess (D2A) service. This is a dynamic, fast-paced role focused on supporting timely, safe, and person-centred discharges from hospital into community settings via Discharge to Assess pathways.
