Key Responsibilities: Complete strengths-based Care Act assessments, reviews and support planning. Manage complex cases, including safeguarding enquiries under the Care Act 2014. Undertake Best Interest Assessments and support Deprivation of Liberty processes. Work closely with health professionals and the wider Multi-Disciplinary Team (MDT) to prevent hospital admissions and promote independence.
