Experienced Adult Social Worker – Review Team We're looking for an experienced Adult Social Worker to join a busy and supportive Adult Social Care Review Team. Key Responsibilities: Complete strengths-based Care Act assessments, reviews and support planning. Manage complex cases, including safeguarding enquiries under the Care Act 2014. Undertake Best Interest Assessments and support Deprivation of Liberty processes. Work closely with health professionals and the wider Multi-Disciplinary Team (MDT) to prevent hospital admissions and promote independence. Participate in the team's duty function, including daily case triage and huddles. Chair multi-agency meetings and contribute to safeguarding, MASH, MARAC and MAPPA processes where required.
