The Intermediate and Frailty Team in Runcorn supports adults who are experiencing reduced mobility, cognitive decline, or increasing care needs. As a Social Work Assistant, you will play a key role in helping individuals maintain independence and access the right support at the right time. You will work alongside Social Workers, health professionals, and community services to deliver high-quality, person-centred support.
Key Responsibilities
Support Social Workers by gathering information, completing initial visits, and assisting with strengths-based assessments.
Help develop and review care and support plans to promote independence and wellbeing.
Liaise with families, carers, and partner agencies to coordinate support and share relevant information.
Assist with arranging services such as home care, reablement, equipment, and community resources.
Contribute to safeguarding enquiries by reporting concerns promptly and accurately.
