We have a variety of contracts depending on your experience within either adults or children’s services, from entry to management. We have an exciting role at Halton Council, to support our hospital discharge team. You will need to have experience of hospital social work, including the Home First approach is desirable. Duties include strengths based assessment and support planning , mental capacity act assessments/ best interest decision making . You will be part of the duty rota. The role is largely onsite due to the nature of the work. You will be joining a friendly team, with good manager presence and support.
