Physiotherapist – Intermediate Care Service ICS Walsall Council/NHS is seeking an experienced Senior Physiotherapist to join their Intermediate Care Team. This role will support individuals to maximise their independence through rehabilitation, rapid response, and reablement services across community and bed-based pathways. Main Duties: Provide physiotherapy assessment and clinical expertise across all Intermediate Care pathways including reablement, rapid response, and rehabilitation beds Support individuals to regain independence through rehabilitation, mobility assessments, and goal-focused interventions Work collaboratively within a multidisciplinary team (MDT) including health and social care professionals Manage and prioritise a varied caseload across community and intermediate care settings Deliver person-centred rehabilitation programmes to support recovery and independence Contribute to discharge planning and prevent unnecessary hospital admissions where possible...
