SEND Case Officer (Preparing for Adulthood 14–25) – Hybrid
An East London Council is seeking an experienced SEND Case Officer to join its Preparing for Adulthood (PfA) Team on an interim basis. The successful candidate will be responsible for managing a caseload of young people aged 14–25 with Education, Health and Care Plans (EHCPs), leading on annual reviews and transition planning while working closely with families, education providers, and multi-agency professionals to support young people as they prepare for adult life.
This is an excellent opportunity for an experienced SEND professional who can confidently manage their own caseload and hit the ground running in a fast-paced Local Authority environment.
The role is offering up to £300 per day (DOE) with an ASAP start. The position is based on a hybrid working arrangement, with one day on-site every Wednesday, which is a mandatory and non-negotiable requirement.
Main Duties:
Manage a caseload of young people aged 14–25 with Education, Health and Care Plans (EHCPs).
Lead on annual reviews, ensuring statutory processes are completed within required timescales.
Coordinate Preparing for Adulthood planning, promoting independence, further education, employment, training, and community inclusion.
Liaise with young people, families, education providers, and multi-agency professionals to ensure positive outcomes.
Coordinate consultations and support placement decisions for post-16 and post-19 educational provision.
Review professional advice and recommendations to ensure EHCPs remain accurate and appropriate.
Maintain accurate case records and update relevant case management systems.
Work collaboratively with education, health, social care, and adult services to support effective transition planning.
Ensure statutory duties under the Children and Families Act 2014 and SEND Code of Practice are met at all times.
Requirements:
Recent experience working within a Local Authority SEND Service as a SEND Case Officer, SEND Caseworker, or SEND Assessment Officer.
Proven experience managing annual reviews within a Local Authority SEND team.
Experience working within a Preparing for Adulthood or Post-16 SEND service.
Strong knowledge of the Children and Families Act 2014, SEND Code of Practice, and post-16 transition pathways.
Ability to manage a busy caseload and work to statutory deadlines.
Excellent communication, organisational, and partnership working skills.
Ability to work independently with minimal supervision.
Must be able to attend the office every Wednesday.
