Adult Social Worker – Step Down Team (Locum) Location: City of Westminster Council, London Pay Rate: Up to £32.00 per hour (Umbrella) Start: ASAP Job Type: Full-Time, Locum City of Westminster Council is seeking an experienced and motivated Adult Social Worker to join our dynamic Step-Down Team. This is an excellent opportunity to make a real impact within a multi-disciplinary, fast-paced service supporting safe and effective hospital discharges into the community. In this role, you will manage a varied and complex caseload, working closely with health and community partners including CIS (Homefirst, Reablement, Community Therapy Team), CLCH, and wider MDT colleagues across the bi-borough. You will be part of a highly collaborative team delivering the Discharge to Assess model, ensuring timely and person-centred Care Act assessments and support planning. You will bring energy, initiative, and problem-solving skills to a well-established team, contributing fresh ideas while maintaining strong practice standards. The role requires excellent communication, strong risk management skills, and the ability to work independently while contributing effectively within a supportive team environment. Key Responsibilities: * Undertake timely Care Act 2014 assessments and support planning following hospital discharge * Complete Mental Capacity Act and safeguarding assessments * Manage a varied caseload within Step-Down / Discharge to Assess pathways * Work collaboratively with MDT partners to ensure safe, effective discharge outcomes * Identify assistive technology and strengths-based solutions to promote independence * Undertake carers’ assessments and support planning * Maintain accurate records and ensure compliance with statutory responsibilities * Participate in duty/urgent response systems and contribute to service development About You: * Minimum 1 year post-qualifying experience in a Local Authority or health setting * Strong knowledge of Care Act 2014, Safeguarding Adults, and Mental Capacity Act 2005 * Experience in multi-disciplinary working and hospital discharge pathways * Confident in risk assessment, caseload management, and care planning * Ability to work independently, prioritise workload, and meet tight deadlines * Social Work England registration and recognised Social Work qualification required This is a fantastic opportunity to join a supportive, high-performing team where your contribution will directly improve outcomes for vulnerable adults and older people transitioning from hospital to community living
